1. I understand that any offer of employment by TLR Contracting Pty Ltd ("the Company") to me is based on the accuracy of the information provided by me, and I declare that such information is, to the best of my knowledge and belief, true and correct. I understand that if any of the information provided by me is false or if I have not disclosed any information to the Company which could affect its decision about whether or not to employ me, my offer of employment may be withdrawn or my employment terminated.

2. 3. I understand the Company’s clients and potential clients collect and use personal information about me from the Company to determine whether or not to use the Company for contracted resource services and to determine whether or not I am suitable for the provision of contracted resource services or other service offerings on behalf of the Company.

5. Whether it be for the consideration for employment, or during the course of my employment should my application be successful, I consent to participate in any applicable suitability tests/checks administered by the Company or its clients and for the results of any tests/checks including medical, drug and alcohol, aptitude tests and criminal record checks) to be disclosed to and used by the Company.
