As part of our recruitment process, a Criminal Conviction and Driver History/Traffic Offence check may be carried out.
I authorise Able Personnel (2024) Ltd to request a copy of my criminal conviction and traffic conviction history through the Ministry of Justice Web Service portal, subject to section 7 of the Criminal Records (Clean Slate) Act 2004, as part of a pre-employment check.
I also authorise the Criminal Records Unit, Ministry of Justice, to release a copy of my criminal conviction, subject to section 7 of the Criminal Records (Clean Slate) Act 2004, to the third party, Able Personnel (2024) Ltd.

DECLARATION AND AUTHORISATION
1. I WILL UPHOLD the standards of work, honesty and reliability expected of any employee, whether temporary or permanent.
2. I AGREE that information given to me by Able Personnel regarding available positions or client companies shall remain absolutely confidential. If accepted by Able Personnel I hereby agree that I will not discuss anything seen or heard in the workplace that the employer / client expects to remain confidential.
3. I AGREE that I will not enter into any private contractual agreement with employers / clients introduced to me by Able Personnel for a period of 12 months from first introduction, unless arrangements are made through the agency. I ALSO AGREE that I will not intentionally introduce any person to an employer / client for a private contractual agreement unless arrangements are made through the agency. Such approaches may lead to legal action.
4. I AUTHORISE Able Personnel to contact any person and seek further information from them which may be relevant to my application for employment. Without limiting the generality of this authorisation, I authorise Able Personnel to obtain any information about me held by credit reference agencies where applicable to a particular position.
5. I AUTHORISE Able Personnel to disclose information which they hold about me to any potential employer relevant to my application for employment.
6. I UNDERSTAND that Able Personnel complies with the Unsolicited Electronic Messages Act 2007. I understand that when registering to use services provided by Able Personnel, I am asked to provide a valid email address and/or mobile phone number. I understand that the supply of an email address and/or mobile phone number will be deemed consent to receive email and/or text messages (SMS or other) from Able Personnel under Part 1 Section 4 (1) by inferred consent. Such email and/or text message will be confined to messages about services that Able Personnel considers may be of interest to me.
7. I UNDERSTAND that my employment may be terminated if, after investigation, any employer discovers that any information which I have provided either in the application or in support of it Is false or misleading.
8. I AUTHORISE Able Personnel to retain any information about me until I advise that I no longer wish to seek employment opportunities through Able Personnel. I understand that Able Personnel might retain non-active information about me on their computer system.
9. I UNDERSTAND that if I have previously been registered with Able Personnel, information from my previous registration (if still available) may be referred to and used again.
10. I CERTIFY that all the information that I have provided to Able Personnel is true, accurate and complete.
Able Personnel will take reasonable precautions in accordance with the Privacy Act 2020 to safeguard your personal information against the loss, misuse
or unauthorised use or disclosure. Personal information held by Able Personnel about you can be accessed by you, and you have the right to request
correction of that information in accordance with the Privacy Act 2020.
